Med Matrix Giveaway: Official Rules

MedMatrix LLC, 198 Maine Mall Road, South Portland, ME 04106 ("Med Matrix" or "Sponsor").

You can enter if, when you enter, you are:

• a legal resident of Maine, New Hampshire or Massachusetts,
• 18 years of age or older, and
• not a current or former Med Matrix patient (you have never had a Med Matrix intake lab visit).

Employees and contractors of Med Matrix, and their immediate family and household members, cannot enter.

This giveaway's entry period starts Monday, October 12 at 12:00 a.m. Eastern Time (ET) and ends at the earlier of Sunday, October 18 at 11:59:59 p.m. ET or the moment 250 eligible entries have been received. The drawing is held Monday, October 19 at about 10:00 a.m. ET.

Go to giveaway.medmatrixusa.com and complete the entry form, including the health-goals survey and the required agreements. This is the only way to enter.

Limit: one entry per person. If you have entered before, you can't enter again.

Grand Prize (1 winner). The Med Matrix Intake Program: a blood test that checks 80+ biomarkers, drawn in person at Med Matrix in South Portland, Maine; an InBody body-composition scan; and a one-hour visit with a Med Matrix provider to review your results and a plan. Approximate retail value (ARV): $1,600.

• Not included: medications, supplements, follow-up visits, membership, travel, or any treatment a provider recommends. Any treatment is the winner's decision and expense.
• The winner must book the blood draw within 30 days of being notified. Massachusetts residents: the blood draw and the first provider visit happen in person in South Portland, Maine.
• If the winner already paid Med Matrix for the Intake Program before the drawing, Med Matrix refunds the amount paid for it instead.
• If the winner cannot complete the Grand Prize for a medical reason, they may name one adult member of their household who meets section 2 to receive it within the same 30 days.

Second Prize (more than one person wins, up to 250 winners). A $300 credit toward the Med Matrix Intake Program. ARV: $300.

• The credit applies only toward the Med Matrix Intake Program, so using it requires buying the Intake Program.
• To claim it, book a free discovery call within 7 days after you are notified. To use it, book your intake lab visit within 5 days after that discovery call.
• No cash value. Not transferable. It cannot be combined with any other credit or discount. It expires if it is not claimed and used within those windows.

Each person can win one prize. Med Matrix may substitute a prize of equal or greater value if a listed prize becomes unavailable.

Winners are chosen in a random computer drawing run by Med Matrix from all eligible entries. No survey answer is used to choose winners: only state of residence, age, past-patient status and the agreement in section 8 decide who can enter, and every eligible entry has the same chance. The Grand Prize is drawn first. Second Prize winners are then drawn at random from the remaining eligible entries, up to 250. If 250 or fewer eligible entries remain, every one of them wins a Second Prize. Med Matrix keeps a record of the drawing.

Entries are capped at 250. The odds of winning depend on the number of eligible entries received. The Giveaway page shows how many spots are left.

The Grand Prize winner is contacted by phone, email and text within 1 business day after the drawing and must reply within 72 hours to accept. If the winner does not reply, is not eligible, or does not follow these rules, the prize is forfeited and an alternate winner is drawn from the remaining eligible entries. Second Prize winners are notified by email, and by text if they agreed to texts, on the day of the drawing. The Grand Prize winner can book their blood draw within 30 days of the drawing. A Second Prize winner claims their credit as described in section 5. Provider visit dates depend on availability; for Massachusetts residents the blood draw and first provider visit happen in person in South Portland, Maine.

By entering, you agree that if you win, Med Matrix may announce your name publicly (for example on its website, social media and ads) without further payment, except where the law does not allow it. Med Matrix never announces anything from your survey answers or any health information.

The Giveaway page shows the most recent Grand Prize winner. For a list of winners, send a stamped, self-addressed envelope to the address in section 1 within 60 days after the drawing.

Med Matrix uses your entry and survey answers to run the Giveaway and to contact you about care, as described in its Privacy Policy at medmatrixusa.com/privacy-policy. Your health answers are never shared with advertising platforms. Message and data rates may apply to texts.

Winners are responsible for any taxes on their prizes. Med Matrix will issue tax forms where the law requires them.

• Med Matrix may disqualify anyone who tampers with the entry process, enters more than once, or breaks these rules.
• Med Matrix is not responsible for lost, late, incomplete or misdirected entries, or for technical failures. If fraud or a technical failure harms the fairness of the drawing, Med Matrix may run the drawing from the eligible entries received before the problem.
• By entering, you release Med Matrix and its employees and contractors from liability for any injury, loss or damage arising from the Giveaway or a prize, except where the law does not allow it.
• Med Matrix's decisions on all Giveaway matters are final.
• These rules are governed by the laws of the State of Maine.
